Follow along

  1. Save the file below as skills/decision-record/SKILL.md, beside the meeting workflow and its references/working-contract.md, which this skill reads.
  2. You rarely call this skill on its own. The workflow page lists when it is used and what it passes on.

# Decision record

Read the [shared contract](../meetings-and-follow-through/references/working-contract.md). Receive the confirmed choice, source evidence, reasons supplied by the source or leader, known alternatives, uncertainties and any permitted record destination.

Capture what was chosen, who confirmed it and when, why the choice was made, relevant alternatives or tradeoffs, open questions and the change that would justify revisiting it. Do not invent a rationale because a record feels incomplete. If the source confirms a decision but omits its reason, label the reason as unstated and ask when that gap affects later use. A tentative discussion remains a candidate decision, with confirmation pending.

Keep the record proportionate to the choice. A modest meeting decision may fit in a short paragraph; a consequential choice benefits from its alternatives and evidence. Include source anchors and dated context so later retrieval can distinguish a prior choice from current facts. Preserve the previous decision when a new choice supersedes it, with the relationship and reason visible.

Prepare the record for the relevant project location, with a descriptive title and keywords a later task can find. Save or update only when the leader's request and available tool authority cover that destination. After a write, reopen the record and check the choice and reason; return the stable location and readback. If no saving route exists, return a complete record draft and proposed destination, without claiming persistence.

## Returns to caller

Decision text, confirmed or pending status, evidence, proposed or verified record location and unresolved reasoning or revisit question. The caller supplies these records to drafts and the meeting artifact.
